Rules, Regulations and Guidance

ODE Forms  

The Office for Exceptional Children (Ohio Department of Education) has revised the state’s Evaluation Team Report (ETR) and Individualized Education Program (IEP) forms. The new forms now have tool tips throughout, providing guidance while educators complete them. The new forms are available online for use. They are required for the 2018-2019 school year.

Essential ETR, IEP and Post-Secondary Transition Livebinder 

The Office for Exceptional Children has developed resources for Essential Individual Education Program (IEP), Evaluation Team Report (ETR) and Post-Secondary Transition for use by educators and parents.
